- Describe the immediate action that should be taken in the event of EACH of the following occurring, stating a possible consequence if the action is not carried out:
(a) a high pressure fuel leak on the main engine;(4)
(b) severe vibration from the main engine;(3)
(c) high cooling water temperature alarm on generator engine.(3)
(a) High pressure fuel leak on the main engine (4 marks)
Immediate action:
- Stop the affected engine or fuel pump immediately.
- Isolate the leaking fuel line or pump and clear away any oil spray.
- Ventilate the area and remove ignition sources.
Possible consequence if not carried out:
- Fuel under high pressure can form a fine spray and ignite, leading to fire or explosion in the engine room.
(b) Severe vibration from the main engine (3 marks)
Immediate action:
- Reduce engine speed and, if necessary, stop the engine.
- Investigate for misfiring cylinder, loose foundation bolts, or damaged propeller.
Possible consequence if not carried out:
- Continued operation may cause structural damage, misalignment, or bearing failure, leading to breakdown of the propulsion plant.
(c) High cooling water temperature alarm on generator engine (3 marks)
Immediate action:
- Reduce load on the generator immediately.
- Check for cooling water supply, pump operation, and sea water inlet strainers.
- If temperature remains high, shut down the generator to prevent damage.
Possible consequence if not carried out:
- Persistent overheating may result in engine seizure, thermal stress, or permanent damage to cylinder liners and heads.
