- To provide clear communication and accountability between those authorising, supervising, and carrying out the work.
(b) SIX items that need to be included on a Permit to Work (6)
- Description of the work to be carried out.
- Exact location of the task.
- Hazards identified and associated risks.
- Precautions required (e.g., ventilation, isolation, PPE, fire watch).
- Duration/validity of the permit (start and expiry time/date).
- Authorisation and signatures (issuing officer, responsible supervisor, and acknowledgement by the worker).
(Other acceptable inclusions: emergency arrangements, equipment isolation details, gas test results, requirement for standby personnel).
